Online Incident Report


DISCLAIMER: The purpose of this site is to allow the Township of Lakewood’s citizens, or any victim of a crime that occurred within the Township limits, to file police reports online.

If the incident you wish to report is an emergency, a crime in progress, or involves any type of injury please do not submit the report online and dial 9-1-1 or call 732-363-0200 ext. 0.

For any other incident that requires a police officer to respond, dial the police non-emergency number 732-363-0200 ext. 0.

Before you proceed, you should be aware that it is a crime to make a false police report and all instances of false reports will be prosecuted. This online reporting system is only to be used to report a limited set of criminal incidents with very specific criteria, listed below.

A Lakewood Police Officer may contact you for additional information related to your case. Please allow 7 business days for reports to be made available. Requests for reports may be picked up in person or by emailing records@lakewoodpolicenj.com. Please include a copy of your valid picture ID, phone number, and the receipt number you will receive upon completion of this form for verification.

Online Reportable Incident Types:
Fraud/ID Theft
Obtaining another person's personal identifying information and using it to fraudulently obtain credit, goods or services.

Example: Someone uses your SSN and/or other identifying information to fraudulently open a credit card account, cell phone account, etc, without your permission.

Lost Property
When property is missing or lost. Including credit cards, ATM cards, and checkbooks.

Example: Property that is missing, left behind in a store, or missing from your home, but is not believed to have been stolen.

Theft
Your property is taken without your permission.

Example: Property known to be stolen, when there are no known suspects. If the value of the property is over $200.00, a report should be filed with an officer, not online. Lost property is not theft.

Vandalism
The act of changing, modifying or defacing public or private property.

Example: Graffiti, knocking down a mailbox, throwing rocks through a window, etc.

Burglary to a Vehicle
The act of unlawfully entering a vehicle.

Example: Someone who makes entry to a vehicle without permission. They may have rummaged through the vehicle and/or taken property.

Vehicle Damage
Damage to a motor vehicle.

Example: Keying, broken windows, or attempts to remove parts. This does not include damage as a result of a vehicle being struck by another vehicle, that would be a Motor Vehicle Crash (see below).

Harassment
Unwanted phone calls of an annoying, harassing or threatening nature. NOT INCLUDING DOMESTIC VIOLENCE RELATED INCIDENTS.

Domestic Violence Related Incidents cannot be reported online. DV reports must be completed by an officer by calling 732-363-0200 ext. 0.

Violation of Child Custody Agreement
When there is a violation of a court ordered visitation/child custody agreement.

Example: When a parent/guardian fails to return the child/children at the agreed upon time in the court agreement or fails to allow the child/children to leave. These incidents can only be reported in Lakewood if they occurred in Lakewood.

I have read the above information and believe this incident to be eligible for on-line reporting. I understand that reporting a false crime is punishable by law.
